Today I came across a website for "Doctors on Demand". This decriptions below is straight form their site:

"

I have not yet signed up to test drive this service but looks like there are 6 "easy" steps (I am being facetious). I am not sure if the use of this telemedicine approach benefits the doctors - when will they take these calls?  How does it benefit the patient - why not call your own physician that you trust and knows you?

I think a more innovative approach would be to establish a video relationship with a doctor of your choice on a different time zone then you are.. Just food for though.
